  • A textured scale-based approach to melanocytic skin lesions in dermoscopy

    Melanoma is the most dangerous and lethal form of human skin cancer and the early detection is a fundamental key for its successful management. In recent years the use of automatic classification algorithms in the context of Computer Aided Diagnosis (CAD) systems have been an important tool, by improving quantification metrics and also assisting in the decision regarding lesion management. This paper presents a novel and robust textured-based approach to detect melanomas among melanocytic images obtained by dermoscopy, using Local Binary Pattern Variance (LBPV) histograms after the Bidimensional Empirical Mode Decomposition (BEMD) scale-based decomposition methodology. The results show that it is possible to develop a robust CAD system for the classification of dermoscopy images obtained from different databases and acquired in diverse conditions. After the initial texture-scale based classification a post-processing refinement is proposed using reticular pattern and color achieving to 97.83, 94.44 and 96.00 for Sensitivity, Specificity and Accuracy.
  • Saúde das Mulheres em Cabo Verde

    Falarmos da saúde das mulheres é falarmos da saúde genésica das populações, já que as mulheres são as principais responsáveis pelo cuidado ao outro. Neste estudo procurámos conhecer a saúde e o bemestar das mulheres em cabo verde, seja a nível físico, seja do ponto de vista emocional, através de um estudo quantitativo e descritivo, dirigido a 104 mulheres cabo-verdianas. Os dados mostram-nos que é necessário investirmos na saúde da mulher em Cabo-Verde, já que as experiências de doença nas mulheres tendem a ser ocultadas e vivenciadas em silêncio, parecendo que a mulher carrega o fardo da resignação e sofrimento, já que as mulheres cabo-verdianas tendem a continuar como de costume, guardando esse facto somente para elas.
  • Evidence-based practice a relevant piece to update knowledge in mental health nursing

    Introduction Web Journal Club as a collaborative learning method, is an effective method to enhance the knowledge base of nursing students, their presentation skills, problem-solving skills and ability to critically appraise literature. Objectives To describe the experience of a online journal club on education of MHN undergraduate students. Methods We implemented a journal club in the online classroom with a total of 24 portuguese undergraduate students enrolled in clinical training of MHN (sixth semester). Over a two-week period, five 2-hour online journal club sessions were conducted in April 2020. During each session, five journal articles were presented synchronously to a live online audience via the Zoom Classroom technology. After all sessions, students were invited by e-mail to complete an anonymous and voluntary online questionnaire via Google Forms. Results All students were all very positive about the journal club sessions and found the opportunity to discuss and reflect on practice issues in depth very helpful. They found the sessions supportive, they helped to bond the group, they learnt a great deal from each others experiences, and they felt that they gained in confidence as a group. Survey results also indicated that few participants experienced technical difficulties during sessions. Conclusions This pedagogical practice enhances gains in the various actors involved: 1) in students, contributing to their learning process and acquisition of competences, articulating research and clinical practice; and 2) lastly, even more indirectly, in people receiving care, since a evidence-based practice ensures safe and quality of nursing care delivery.
  • Personal narratives about death and dying: a case study with nursing students

    This study aimed to qualitatively explore students' meanings of a significant death or dying experience described in the first-person: psycho-emotional and spiritual life reflections. Using a narrative-reflection approach, we performed a multiple case study. Ten nursing students from a 3 rd-year palliative nursing course were selected based on the richness of their experiences. Topics such as the timing of the loss, the manner of death, role changes after the loss, and adjustment patterns were discussed. The role of the narrative is clearly multi faceted – contributing to the development of a student's identity, providing comfort, and helping to make sense of a 'senseless' event.
  • Impact of the COVID-19 Pandemic on Adherence to Exercise Prescription: The case of Cardiac Rehabilitation Programs

    Cardiac Rehabilitation Programs (CRPs) are an important tool of secondary prevention and their implementation within health services, despite the uneven geographical distribution, has been receiving attention from decision-makers in recent years. Adherence to the CRPs is one of the great challenges faced by the multidisciplinary team, and there are several strategies to maintain adherence, particularly in CRP-Phase III, which occurs outside the hospital environment. One of the strategies followed is the use of remote performance monitoring and recording of possible alert symptoms. With the pandemic due to COVID-19, these challenges have become even more evident as Phase II programs have been suspended, thus increasing the importance of the home-based CRPs. In this work, the results of a study aiming to understand the impact of the pandemic on adherence to the prescription of exercise and the perception of patients regarding the effects of physical activity on health conditions are shown. The results indicate that the pandemic did not have a major effect on the adherence to home-based exercises, in particular, in patients undergoing programs using a telemonitoring system. Moreover, the perception of the importance of physical activity for health and well-being was reinforced in the context of the pandemic.

    O trabalho forma parte da apresentação realizada no "Simposio de Terapia Ocupacional en tiempos de pandemia", celebrado en Argentina no mês de novembro de 2021, cujo objetivo era abordar as diferentes estratégias apresentadas pelos participantes, docentes universitários de diferentes Instituições de Ensino Superior na área da Terapia Ocupacional e de diferentes países, no sentido de promover o ensino de qualidade, apesar da Pandemia.

    O terapeuta ocupacional em saúde mental atua como promotor do empoderamento, justiça ocupacional e inclusão comunitária. Desta forma, conclui-se que a Terapia Ocupacional deve intervir junto de pessoas com experiência em doença mental por uso de substâncias psicoativas, de forma a trabalhar na reorganização do dia a dia destas pessoas.
  • A Conversational Agent for Promoting Physical Activity Among COPD Patients

    Chronic Obstructive Pulmonary Disease (COPD) is one of the most prevalent diseases in the world, affecting respiratory performance of many people, limiting the airflow and is not fully reversible. It is a clinical syndrome characterized by chronic respiratory symptoms, structural pulmonary abnormalities or impairment of lung function. In order to help people with this disease, we propose an innovative personalized mHealth coaching platform that will address patient preferences and contextual factors – the OnTRACK platform. This platform is composed of a mobile application for patients, a web platform for healthcare professionals – and a conversational agent (or chatbot), named “Hígia”, which acts as an alternative interface between patients and the platform. This conversational agent includes several of the main functionalities already available in OnTRACK’s smartphone app, complementing and extending it. It allows consulting prescription information in a multitude of ways, getting and setting all personal data, inserting physical activity measurements, and obtaining historical data on physical activity and prescriptions, among others. The evaluation of the conversational agent yielded encouraging results, with users reporting being happier, more motivated, dedicated and confident when interacting with the systems using their voice, while allowing the development team to identify topics for improvement.
  • Addressing spiritual needs, facilities and barriers in portuguese palliative care: a cross-sectional survey

    Background: Spiritual care is an essential component of Palliative Care (PC) and a dimension of holistic care that preserves dignity and helps sick people to find meaning in suffering and in life. The knowledge about its implementation is still scarce in the Portuguese context. This study aimed to evaluate the facilities, barriers and needs for spiritual care in PC. Method: We conducted a descriptive cross-sectional survey of 180 health professionals in the period November 2018 to September 2019 at PC units. The data were collected through an online questionnaire published on social networks, after approval by an Ethics Committee. Sociodemographic and professional data, spiritual beliefs and knowledge about the objectives and purposes of PCs were collected. These were evaluated using an instrument consisting of 26 statements of dichotomous response (true/false). Results: The average age of participants was 42.42 years (SD=11.35), most are female (82.2%), nurses (43.9%), and 70% work in the health field. The total sample has worked on average for 15.67 ± 10.28 years and more than half (58.9%) perform functions in palliative care. Most identify with a religious belief (87.8%), attaching great importance to spiritual/religious conviction (45% of participants). The results suggest that empathy and the therapeutic use of silence (82.2%) are facilitators of spiritual care. The key barriers refer to the lack of expert professionals and the organization of care (36.1%). The results also highlight the need for specific training in the area of spirituality, hope and intervention strategies (53.9%). Conclusion: This study has provided insight into spiritual care in PC in Portugal. Future studies are necessary to investigate the effects of spiritual care more fully, and to develop outcome measurements that appropriately capture the effects of the variety of spiritual care practices.
