Abstract(s)
O presente relatório foi realizado com o objetivo de ser feita uma análise crítico-reflexiva sobre
o percurso que desenvolvi para a aquisição de competências comuns e específicas do
Enfermeiro Especialista e Mestre em Enfermagem de Saúde Comunitária e Saúde Pública.
Na primeira parte será realizada uma breve análise crítica e reflexiva sobre os ensinos clínicos
que decorreram ao longo de todo o percurso académico, nomeadamente na Unidade de
Saúde Pública do Médio Tejo e a Unidade de Cuidados na Comunidade , onde
adquiri e desenvolvi competências comuns e específicas do Enfermeiro especialista. Numa
segunda parte, irei descrever o Projeto de Intervenção Comunitária que desenvolvi, com base
na metodologia do Planeamento em Saúde, sendo abordadas as etapas: diagnóstico de
situação, definição de prioridades, fixação de objetivos, escolha das estratégias, elaboração
e execução do Projeto e a respetiva avaliação.
O Projeto de Intervenção Comunitária designado “Empoderamento Comunitário de idosos
polimedicados para a adesão ao regime medicamentoso” refere-se a uma População de 21
idosos do concelho de . Foi realizada uma revisão integrativa da
literatura sobre a temática de forma a sustentar a prática com uma base de evidência
científica.
O envelhecimento populacional é uma realidade a nível mundial, que aumenta a prevalência
de doenças crónicas entre a população e, por sua vez, o recurso a múltiplos fármacos para o
seu controlo. Este facto, aliado à progressiva e gradual deterioração da vertente física e
cognitiva verificada nos idosos, colocam este grupo vulnerável. A segurança do doente, em
geral, e a segurança na medicação em específico, originam um problema de saúde pública e
cabe ao Enfermeiro Especialista em Enfermagem Comunitária e de Saúde Pública intervir
nesta problemática.
Neste Projeto procedeu-se à avaliação do nível de Empoderamento Comunitário, através da
aplicação do Modelo de Avaliação, Intervenção e Empoderamento Comunitário. Para
avaliação da Gestão Comunitária foi aplicada a Escala de Avaliação do Empoderamento
Comunitário e a escala Medida de Adesão aos Tratamentos. Os dados obtidos, juntamente
com os resultados da revisão da literatura, permitiram a definição de diagnósticos de
Enfermagem, desenvolver intervenções com a comunidade de idosos e melhorar a sua
adesão ao regime medicamentoso. Numa fase posterior, perspetiva-se a expansão deste
Projeto idealmente a toda área de abrangência da Unidade Local de Saúde do Médio Tejo.
This report aims to provide a critical and reflective analysis of the journey undertaken to acquire the common and specific competencies of a Master's Degree Nurse specialising in Community Health and Public Health Nursing. The first part will be a brief critical and reflective analysis of the clinical training that took place throughout my academic career, namely at the Médio Tejo Public Health Unit and the Community Care Unit, where I acquired and developed a specialist nurse's common and specific competencies. In the second part, I will describe the Community Intervention Project developed by me, based on the Health Planning methodology, covering the following stages: diagnosis of the situation, definition of priorities, setting of objectives, choice of strategies, preparation and implementation of the project and its evaluation. The Community Intervention Project entitled “Community Empowerment of polymedication elderly people to adhere to their medication regime” covered 21 elderly people from the municipality of . An integrative review of the subject's literature was carried out to support the practice with a scientific evidence base. An ageing population is a reality all over the world, leading to an increase in the existence of chronic diseases among the population and, in turn, the use of multiple drugs for its control. This, coupled with the gradual and progressive physical and cognitive decline experienced by the elderly, makes them a vulnerable group. Patient safety in general, and medication safety in particular, is a public health problem and it is up to the Nurse Specialising in Community and Public Health Nursing to intervene in this issue. This project assessed the level of Community Empowerment by applying the Community Assessment, Intervention and Empowerment Model. The Community Empowerment Assessment Scale and the Treatment Adherence Measure scale were used to assess community management. The data obtained, together with the results of the literature review, allow the definition of nursing diagnoses, developing interventions with the elderly community and improve their adherence to the medication regime. At a later stage, the project is expected to be expanded, ideally to the entire area covered by the Médio Tejo Local Health Unit.
