Abstract(s)
O presente relatório surge no âmbito do mestrado e especialização em Enfermagem de Saúde Mental e
Psiquiátrica e pretende descrever e refletir sobre o percurso de desenvolvimento e a consolidação de
competências comuns e especializadas do enfermeiro especialista e mestre em Enfermagem de Saúde
Mental e Psiquiátrica.
O relatório integra três partes. A primeira parte faz referência a uma descrição breve dos contextos da prática
clínica em ensino clínico, numa Unidade de Cuidados na Comunidade, numa unidade de internamento de
doentes agudos, e em contexto de respostas diferenciadas num Centro de Respostas Integradas, numa
Equipa Técnica Especializada de Tratamento.
Na segunda parte é realizado uma descrição e reflexão das atividades e das aprendizagens nos diferentes
contextos de prática clínica e a sua contribuição para o desenvolvimento e otimização de competências
comuns e especializadas em Enfermagem de Saúde Mental e Psiquiátrica.
Na terceira parte deste relatório apresenta-se um projeto de melhoria contínua da qualidade dos cuidados
de enfermagem que surge a partir de um problema experienciado na prática clínica.
Receber um diagnóstico de cancro, significa entre outras coisas, que o futuro é incerto. O sentimento de
esperança é, por sua própria natureza, ligado ao futuro, e expectativas ligadas com o futuro afetam
significativamente a sensação de bem-estar. A esperança é o máximo recurso comum após um diagnóstico
de cancro e um importante fator que contribui para o bem-estar, a sobrevivência, e qualidade de vida e, é,
portanto, um importante aspeto do cuidado da pessoa com cancro. A ansiedade é um dos diagnósticos mais
frequentes, que acompanha as pessoas com cancro. A esperança também é um fator de proteção para
ajudar as pessoas a adaptarem-se à ansiedade e a ajustarem-se ao desconforto psicológico.
O projeto de melhoria contínua da qualidade dos cuidados de enfermagem intitula-se “Promoção da
esperança e gestão da ansiedade nas mulheres com cancro da mama” e foi construído com base no ciclo de
Deming e nos enunciados descritivos dos Padrões de Qualidade dos Cuidados Especializados em
Enfermagem de Saúde Mental. A partir da identificação das necessidades de um contexto específico,
elaborou-se o diagnóstico da situação, planificaram-se as intervenções, e implementou-se um programa de
intervenção psicoterapêutica em cinco sessões individuais. No culminar do primeiro ciclo de implementação,
explanam-se os resultados obtidos com a respetiva análise, sustentada pelo conhecimento científico, numa
perspetiva de melhoria dos cuidados especializados de enfermagem, perante uma necessidade de resposta
inexistente.
Na otimização das competências desenvolvidas, este relatório vem permitir a reflexão síntese de todo o
percurso, da eficácia da dinâmica ensino-aprendizagem-ação, da aplicação do conhecimento científico
fundamentada nos referenciais teóricos de enfermagem e nos documentos reguladores da Enfermagem
Especializada em Saúde Mental e Psiquiátrica, de forma a sustentar uma prática de qualidade, segura e de
excelência.
This report is part of the master's degree program, which specialises in mental health and psychiatric nursing. It aims to describe and reflect on the development and consolidation of standard and specialised competencies of Mental Health and Psychiatric Nursing. The report has three parts. The first part refers to a brief description of the contexts of clinical practice in clinical teaching, in the community in a Community Care Unit, inpatient in an acute inpatient unit, and in the context of differentiated responses in an Integrated Response Centre, in a Specialised Technical Treatment Team. The second part describes and reflects the activities and learning in the different clinical practice contexts and their contribution to developing and optimising common and specialised competencies in Mental Health and Psychiatric Nursing. Receiving a cancer diagnosis means, among other things, that the future is uncertain. The feeling of hope is, by its very nature, linked to the future, and expectations linked to the future significantly affect the feeling of well-being. Hope is the most common resource after a cancer diagnosis and an essential factor contributing to well-being, survival and quality of life, and is therefore an important aspect of caring for people with cancer. Anxiety is one of the most frequent diagnoses that accompany people with cancer. Hope is also a protective factor to help people adapt to anxiety and adjust to psychological discomfort. The project to continuously improve the quality of nursing care is entitled ‘Promoting hope and managing anxiety in women with breast cancer’ and was built on the Deming cycle and the descriptive statements of the Quality Standards for Specialised Care in Mental Health Nursing. After identifying the needs of a specific context, a diagnosis of the situation was drawn up, interventions were planned, and a psychotherapeutic intervention programme was implemented in five individual sessions. At the end of the first implementation cycle, the results obtained are explained with the respective analysis, supported by scientific knowledge, with a view to improving specialised nursing care in the face of a non-existent need for a response. In order to optimise the skills developed, this report provides a summary reflection of the whole process, the effectiveness of the teaching-learning-action dynamic, the application of scientific knowledge based on theoretical nursing references and the regulatory documents for Specialised Mental Health and Psychiatric Nursing, to support a quality, safe and excellent practice.
