Estimulação Cognitiva na Doença Mental Crónica: Intervenção nos Domínios da Atenção e Memória: Relatório de Estágio

O presente relatório de estágio descreve e analisa criticamente o percurso de desenvolvimento de competências especializadas em Enfermagem de Saúde Mental e Psiquiátrica (ESMP), realizado no âmbito do Mestrado em Enfermagem de Saúde Mental e Psiquiátrica da Escola Superior de Saúde de Leiria. O itinerário formativo percorreu três contextos assistenciais distintos: a Unidade de Cuidados na Comunidade de Ourém (ULS Médio Tejo), o Serviço de Internamento de Psiquiatria de Peniche (ULS Oeste) e o Departamento de Psiquiatria e Saúde Mental das Caldas da Rainha.
Ancorado na Teoria das Transições de Afaf Meleis e no modelo de Relações Interpessoais de Hildegard Peplau, o percurso focou-se na transição de papel do enfermeiro de cuidados gerais para o patamar de enfermeiro especialista. A prática clínica foi sustentada por intervenções de matriz cognitivo-comportamental, psicoterapêutica e socioterapêutica, centradas na capacitação do utente e da família para o processo de recovery.
Atendendo às necessidades identificadas, foi implementado um Programa de Melhoria Contínua da Qualidade dos Cuidados de Enfermagem (PMCQCE) focado na estimulação cognitiva. A operacionalização deste programa permitiu sistematizar a intervenção especializada, utilizando a estimulação cognitiva como ferramenta para otimizar a funcionalidade e a autonomia dos utentes. Os resultados obtidos evidenciam ganhos em saúde mental, nomeadamente na manutenção de capacidades cognitivas remanescentes e no reforço da aliança terapêutica através do uso terapêutico do eu.
Em conclusão, este relatório demonstra a integração de saberes teóricos e práticos, validando a aquisição das competências comuns e específicas exigidas pela Ordem dos Enfermeiros. O percurso revelou-se transformador, consolidando uma identidade profissional assente na peritagem clínica, na ética e na busca incessante pela excelência e segurança dos cuidados em saúde mental.
This internship report describes and critically analyzes the development of specialized competences in Psychiatric-Mental Health Nursing (PMHN), conducted as part of the Master’s Degree in Psychiatric-Mental Health Nursing at the School of Health Sciences of Leiria. The clinical pathway encompassed three distinct healthcare settings: the Community Care Unit of Ourém (ULS Médio Tejo), the Psychiatric Inpatient Service in Peniche (ULS Oeste), and the Department of Psychiatry and Mental Health in Caldas da Rainha. Grounded in Afaf Meleis’ Transitions Theory and Hildegard Peplau’s Theory of Interpersonal Relations, this journey focused on the role transition from general care nurse to the level of nurse specialist. Clinical practice was supported by cognitive-behavioral, psychotherapeutic, and sociotherapeutic interventions, centered on empowering the user and their family throughout the recovery process. Addressing the identified needs, a Nursing Care Quality Continuous Improvement Program (NCQCIP) focused on cognitive stimulation was implemented. The operationalization of this program allowed for the systematization of specialized care, using cognitive stimulation as a strategic tool to optimize the users' functionality and autonomy. The results demonstrate significant mental health gains, particularly in maintaining remaining cognitive capacities and strengthening the therapeutic alliance through the therapeutic use of self. In conclusion, this report demonstrates the integration of theoretical and practical knowledge, validating the acquisition of the core and specific competencies required by the Order of Nurses. The process proved to be transformative, consolidating a professional identity based on clinical expertise, ethics, and the pursuit of excellence and safety in mental health care.
